If you have additional health concerns that require you to receive more frequent or intensive treatment than what is covered by our dental care membership plan, you may want to consider signing up for Aesthetic Dentistry Associates’s Periodontal Membership Plan. This discount plan will enable you to receive the additional care you need without worrying about the cost of your treatment. The Periodontal Membership Plan offers all the same benefits as our standard Dental Care Membership Plan. You will have an initial feel of $99 when you enroll, which will cover your necessary examinations and X-rays. After your enrollment, you will immediately be eligible for your membership benefits.
There are two membership levels within this plan:
- $39.50 per month for 3 hygiene visits per year
- $48.50 per month for 4 hygiene visits per year
The membership level necessary to maintain a healthy mouth will be determined during your initial examination, and will be re-evaluated at each subsequent yearly examination.
Benefits of the Periodontal Membership Plan include:
- 3-4 hygiene visits per year
- 2 dental exams per year
- All necessary X-rays
- 2 emergency visits per year during normal business hours
- Fluoride treatment (1 per adult per year, 2 per child per year)
- 15% discounts on fillings, grounds, and other general services
- 10% discounts on implants, dentures, cosmetic procedures, and sedation services
- 20% discount on orthodontics
*Periodontal infection control services may be necessary before entering into your maintenance plan. These services are covered at a 15% discount, and will be determined at your initial examination.

Children under age 19 and full-time students up to age 23 are eligible for child membership rates. Coverage of a child age 19 will be continued if the child is incapable of self-sustaining employment due to developmental disability or physical handicap.
